  • PATIENT’S RIGHTS

    • Quality healthcare regardless of race, color, national origin, religion, sex, marital status, and age

    • Reasonably informed participation in healthcare decisions

    • Complete information on condition, treatment plan, and outlook for recovery

    • Informed consent for all procedures

    • Emergency treatment regardless of circumstances

    • The name of physician or other practitioner with primary responsibility for care

    • Respectful, courteous, and dignified treatment

    • Consultation and communication

    • Complain or compliment without the fear of retaliation or compromise of access to quality care

    • Refusal of treatment. Your physician must inform you of the consequences of refusal. You are responsible for your actions if you refuse treatment or do not follow the practitioner's instructions.

    • Care that recognizes psychosocial, spiritual, and cultural needs, and the opportunity to exercise cultural or spiritual beliefs that do not harm others or interfere with medical treatment

    • Knowledge of hospital regulations

    • A copy of billing showing charges for each service received

    • Access to medical records and an opportunity to obtain copies at a reasonable cost or to challenge the accuracy of records. Private pay patients have the right to restrict research-related access to their medical records.

    • Privacy and the assurance that details of condition and treatment may not be disclosed at any time except to those entitled to receive the information. Your medical record will not be shown or released to anyone not connected with DLSUMC without your permission, unless under court order or legal mandate.

    • Refusal of experimentation and participation in research

    • Participation in consideration of ethical issues related to your care. These issues may include resolution of conflicts, withholding or withdrawal of life-sustaining treatment, and participation in investigational studies and clinical trials.

    • Personal safety.

  • PEDIATRIC PATIENTS HAVE THE RIGHT TO:

    • Have a parent or guardian stay with them, except in certain areas such as the operating room, intensive care unit, or during certain procedures, as determined by their care givers

    • Opportunity for play, recreation, and education suited to their age, developmental level, and condition, and an environment designed, furnished, staffed, and equipped to meet their needs

    • Care responsive to the physical, emotional, and developmental needs of children and families

  • PATIENTS’ RESPONSIBILITIES

    Your diligence in following through on these responsibilities will help us provide the best possible care.


    • Follow hospital rules and regulations.

    • Provide assurance that financial obligations are met.

    • Provide DLSUMC and its Clinics with accurate health insurance information and/or make other appropriate arrangements for reimbursement of services rendered.

    • Respect the rights of other patients, families, visitors, and staff.

    • Provide complete and accurate information about matters pertaining to your health care.

    • Request for more information from medical or nursing staff if you don't understand your illness or treatment.

    • Cooperate with care givers to properly implement your treatment plan.

    • Report changes in your medical condition after discharge.
