A Letter of Authorization (LOA) is needed within 24 hours upon admission. Coordinate with your HMO for the assurance of hospitalization coverage.
Note: For elective cases/scheduled procedures, LOA is required prior to admission.
For HMO concerns, proceed to Corporate Clinical Services
(3/F Medical Arts Center/MAC), Monday-Saturday, 7:00 AM-5:00 PM.
For PhilHealth members/dependents:
Inquiry on PHIC status and requirements may be done at our Claims Section (Ground Floor, MAC).
Medicines for inpatients and the Hospital formulary:
- Please be informed that ONLY PNF/PNDF (Philippine National Drug Formulary) medicines will be covered by PHIC/HMO insurance based on PhilHealth Circular No. 2006-0386 and DOH Administrative Order No. 2016-0034.
- Should your attending physician prescribe non PNF/PNDF medicines, you may be asked to purchase them at the DLSUMC Pharmacy.

REMINDERS:
- Processing of discharge is from 8:00 AM to 11:00 AM.
- Final bill has to be settled on or before 1:00 PM.
Note: Discharge beyond 1:00 PM may pose additional charges.
All payments, including doctor’s professional fees, shall be made ONLY at the Hospital Cashier.
DISCHARGE PROCEDURE:
1. Patients will be informed by their respective nurses-in-charge once their attending physician finalizes the discharge order.
2A. For patients with no PhilHealth and HMO cards, please proceed to Step 3.
2B. For patients with PhilHealth, please proceed to the Claims Section (G/F, MAC) for benefit computation. Kindly submit the following requirements:
- For patients who have undergone a certain operation, get a copy of the Records of Operation from the nurse-in-charge and submit it together with other requirements.
- For patients who have undergone delivery either via cesarean section or normal spontaneous delivery, get the PhilHealth form (CF3) and Delivery Room Record from the nurse-in-charge.
- For patients with health cards, submit the Letter of Authorization (LOA) to the Billing Section (G/F, MAC), if not yet secured since admission.
3. Proceed to the Billing Section for the final billing statement.
Note: To avail of the senior citizen discount, the patient should present his/her ID card. To avail of a PWD discount, present the PWD ID for verification of validity.
4. Proceed to the Hospital Cashier (G/F, MAC) for payment of hospital bills. A clearance slip will be issued by the cashier once the bills are settled.
5. Give the white clearance to the nurse-in-charge.
6. Get the Discharge Instruction Form and prescribed home medication from the nurse and fill out the Patients’ Feedback Form.
7. Give the pink and blue clearance to the security guard on duty upon exit.

Credit Agreement Accreditation Program (CAAP) Partner Companies
The DLSUMC Credit Agreement Accreditation Program (CAAP) is a partnership that allows accredited companies’ employees, members, and qualified dependents to access DLSUMC’s services and facilities. Designed to address the immediate and specific healthcare needs of organizations, CAAP offers timely support, especially during uncertain situations.
At least two (2) valid identification cards are needed upon admission.

PAGPAPALIT NG LINEN
Ang pagpapalit ng mga linen (kumot, kobre kama, at punda) ay tuwing umaga.
Ang mga karagdagang request para sa linen ay may katumbas na halagang idadagdag sa inyong hospital bill.

- Mangyari lamang na paghiwalayin ang inyong basura at itapon sa tamang basurahan:

MGA KARAGDAGANG PAALALA

MGA PATAKARAN SA PAGLABAS (DISCHARGE) SA OSPITAL
Ipagbibigay-alam ng nars sa pasyente kung mayroon nang discharge order galing sa doktor.
2A Sa mga pasyenteng walang PhilHealth at HMO card, magpatuloy sa Step 3
2B Sa mga pasyenteng may PhilHealth, magpunta sa Claims Section (G/F, MAC) para makuha ang komputasyon ng benepisyo mula sa PhilHealth. Ibigay ang mga sumusunod:
- 2B.1 Sa mga pasyenteng sumailalim sa operasyon, kunin ang record ng operasyon sa nakatalagang nars at isumite ito kasama ng iba pang requirements.
- 2B.2 Sa mga pasyenteng nanganak (cesarean o normal), kunin ang PhilHealth form (CF3) at Delivery Room Record sa nakatalagang nars.
2C. Sa mga pasyenteng may health card, ibigay ang Letter of Authorization o LOA sa Billing Section (G/F, MAC), kung hindi pa ito naibibigay mula nang ma-admit.
3. Pumunta sa Billing Section para sa kopya ng kabuuang bayarin. Paunawa: Upang makakuha ng senior citizen discount, ipakita ang senior citizen ID card. Upang makakuha ng PWD discount, ipakita ang PWD ID para sa beripikasyon.
4. Pumunta sa Hospital Cashier (G/F, MAC) upang magbayad ng kabuuang bayarin. Ibibigay ng kahera ang clearance slip.
5. Ibigay ang puting clearance slip sa nakatalagang nars. Kunin sa nars ang Discharge Instruction Form at ang mga niresetang gamot. Magsagot ng Patients’ Feedback Form.
6. Ibigay ang pink at ang blue clearance slips sa gwardya bago lumabas ng ospital.
