APPLICATION REQUIREMENTS
DLSUMC Medical Staff Credentials Checklist
- Accomplished application form
- Passport size photo (2 pcs)
- Curriculum Vitae
- Letter of intent addressed to the Medical Director
- Declaration of good standing
- Affirmation of competence from two (2) doctors
- Delineation of clinical privileges
Photocopy of the following:
- Specialty board certification
- Sub-specialty board certification (if applicable)
- Certification of fellowship training (if applicable)
- Certificate of residency training
- Certificate of internship
- Medical diploma
- Physician licensure certificate
- Current PRC Identification Card
- Narcotic (S2) license card (if possible)
- PhilHealth Accreditation ID
- Tax Identification Number (TIN) / Certificate of Registration (COR)
Submit all requirements and credentials to the Office of the Medical Director. Please bring original certificates for cross-reference.
